Hurdle values complaints and feedback from people with disability, providers, and regulators to ensure people are treated fairly when they use our services. Complaints and/or feedback can be received verbally, in writing or in other way that suits the person completing the form. Families, carers and advocates can also submit the form on behalf of the person with disability that requires support.
